Our teams pair traditional field investigation with modern open-source and digital methods, so you get the full picture without the noise — documented, sourced, and ready for whatever comes next.

Built for Insurance Carriers & Defense Counsel // Insurance Fraud · SIU

Defensible evidence for the claims that don't add up.

Insurance fraud is INTEGRIT's core discipline. For carriers, TPAs, self-insured employers, and defense counsel, we deliver surveillance, claimant intelligence, and SIU support built to survive claims review, examinations under oath, and trial — handled by one accountable point of contact and worked the same way on every file, in every state.

Comp claims
01

Workers' Compensation

Surveillance and activity verification that tests a comp claim against what the claimant actually does — capacity versus stated restrictions, undisclosed work, and the inconsistencies that decide a file.

Workers' compensation is where INTEGRIT does its most volume, and where surveillance pays for itself fastest. We document a claimant's real-world activity and functional capacity against the alleged injury and restrictions, discreetly and on a timeline that fits the claim.

What's involved

  • Covert surveillance timed to the claimant's routine and medical appointments
  • Activity and capacity documentation measured against stated restrictions
  • Identification of undisclosed employment or secondary income
  • Social media and OSINT corroboration integrated with the field record
  • Coordination with the adjuster and defense counsel on scope and timing

Typical engagements

  • Questionable or long-duration temporary-disability claims
  • Claims with inconsistent medical reporting or restrictions
  • Suspected secondary employment while on benefits
  • Pre-IME, pre-hearing, or pre-settlement verification

What you receive

  • A chronological investigator report tied to the claim issues
  • Time- and date-stamped video and photo exhibits
  • A findings summary for the adjuster, SIU file, and counsel
Long-duration claims
02

Disability & LTD

Functional-capacity documentation over time on short- and long-term disability claims — the sustained, dated record carriers need on the claims that run longest and cost most.

Disability and LTD claims carry the longest tail and the largest reserves. INTEGRIT builds a dated record of a claimant's activity and functional capacity over time, so the carrier's decision rests on documented reality rather than self-report.

What's involved

  • Multi-day and periodic surveillance to establish patterns, not snapshots
  • Activity documentation against the claimed functional limitations
  • Verification of reported activity level, travel, and work
  • Social media and OSINT review for activity inconsistent with the claim
  • Re-verification at review or renewal milestones

Typical engagements

  • STD/LTD claims with subjective or hard-to-verify conditions
  • Claims approaching the own-occupation to any-occupation transition
  • Periodic re-certification and continuing-disability reviews
  • Claims with activity inconsistent with reported limitations

What you receive

  • A dated, chronological activity record with investigator observations
  • Time-stamped video and photo exhibits
  • A summary suitable for claims review, IME referral, or counsel
Auto · premises · GL
03

Liability & Bodily Injury

Scene, witness, and claimant investigation on auto, premises, and general-liability claims — including the staged-accident and exaggeration indicators that separate real injuries from manufactured ones.

Bodily-injury and liability claims turn on facts gathered early. INTEGRIT investigates the scene, the witnesses, and the claimant so the carrier and defense counsel know what actually happened — and can recognize staging, exaggeration, or organized activity when it's there.

What's involved

  • Scene investigation, photography, and measurement
  • Witness identification, canvass, and recorded statements
  • Claimant background, prior-claims history, and injury-consistency review
  • Staged-accident and organized-fraud indicator analysis
  • Surveillance of claimants alleging ongoing injury or limitation

Typical engagements

  • Auto accident BI claims with soft-tissue or disputed injuries
  • Premises-liability slip, trip, and fall claims
  • General-liability claims against insured businesses
  • Suspected staged collisions or organized claim activity

What you receive

  • A liability investigation report with scene documentation
  • Witness statements and exhibits ready for counsel
  • A claimant findings summary with surveillance exhibits where applicable
Compensability
04

AOE / COE & Statements

Arising-out-of and course-of-employment investigations with timeline reconstruction and recorded statements — the compensability facts a carrier needs before a claim is accepted or denied.

Compensability questions have to be answered fast and on the record. INTEGRIT reconstructs the timeline of an alleged injury, secures recorded statements from the claimant, employer, and witnesses, and documents whether the injury arose out of and in the course of employment.

What's involved

  • Timeline reconstruction of the alleged injury event
  • Recorded statements from claimant, employer, and witnesses
  • Employment, duty, and location verification
  • Review of prior injuries and claims history
  • Clear documentation of the compensability facts for the file

Typical engagements

  • Late-reported or unwitnessed injury claims
  • Injuries alleged off-site, after hours, or during breaks
  • Disputes over whether the activity was work-related
  • Claims filed shortly after discipline, layoff, or termination

What you receive

  • An AOE/COE report with the reconstructed timeline
  • Recorded statements and supporting exhibits
  • A compensability findings summary for the adjuster and counsel
Special Investigations
05

SIU Program Support

Referral triage, desktop and field investigation, claims-history review, and reporting built to satisfy state anti-fraud plan requirements — a true extension of your SIU, not just a surveillance vendor.

INTEGRIT works as an extension of carrier SIU units: taking referrals, triaging them, running the desktop and field work, and delivering reporting that meets state anti-fraud plan and regulatory requirements. One point of contact handles your referrals the same way, every time, nationwide.

What's involved

  • Referral intake and triage against SIU red-flag criteria
  • Desktop investigation: claims-history, database, and public-records review
  • Field investigation and surveillance as the referral warrants
  • Documentation structured to state anti-fraud plan and reporting standards
  • Testimony and case support through EUO, hearing, and trial

Typical engagements

  • Carriers and TPAs needing overflow or specialized SIU capacity
  • Self-insured employers without an in-house SIU
  • Fraud-plan compliance and reporting support
  • Multi-state referrals requiring consistent handling

What you receive

  • Consistent SIU-grade investigation reports on every referral
  • Exhibits and documentation ready for regulatory referral where warranted
  • A single accountable point of contact across all your files
Digital intelligence
06

Claimant Social Media & OSINT

Lawful, documented digital intelligence on claimants — activity, travel, employment, and associations — preserved for the file and integrated with the field investigation.

A claimant's digital footprint often tells the story the claim doesn't. INTEGRIT's analysts lawfully collect, verify, and preserve open-source and social media intelligence on claimants, then integrate it with surveillance so the file carries a complete, corroborated picture.

What's involved

  • Social media activity, travel, and lifestyle review consistent with the claim
  • Employment, business, and side-income indicators
  • Associations and connections relevant to organized or collusive activity
  • Forensically sound capture and preservation for the file
  • Integration with surveillance for timing, location, and corroboration

Typical engagements

  • Pre-surveillance intelligence to time and target field work
  • Claims where activity level is in question
  • Suspected undisclosed employment or business activity
  • Organized-fraud and provider-pattern review

What you receive

  • A sourced digital intelligence brief with preserved exhibits
  • Clear separation of verified fact from inference
  • Actionable leads for surveillance and the SIU file

02 — Open-Source Intelligence

The public record, read properly.

We collect and analyze open-source and digital footprints — social, public records, breach data, and beyond — to surface connections, verify identities, and map risk. Structured collection, careful analysis, and sourcing you can trace back.

  • Digital Footprinting
  • Asset & Link Analysis
  • Identity Verification
  • Threat Mapping

A remarkable amount of decisive information is already public — if you know how to find it, verify it, and connect it. Our analysts run structured open-source collection across social platforms, public records, corporate filings, court dockets, and lawfully obtained breach data to build a sourced intelligence picture without ever touching private systems.

What's involved

  • Digital-footprint mapping across social, forums, and the open web
  • Public-records, corporate-registry, and court-docket research
  • Identity resolution and alias, asset, and relationship link analysis

03 — Fraud & Financial Investigations

Follow the money, cleanly.

When something doesn't add up, we trace it. Asset identification, financial-history reconstruction, and fraud analysis that gives you — and your counsel — a documented basis for recovery, litigation, or a quiet resolution. For matters that require forensic accounting, INTEGRIT retains Certified Fraud Examiners (CFEs) through our 400+ investigator alliance to conduct the forensic examination alongside our investigators.

  • Asset Searches
  • Fraud Analysis
  • Forensic Accounting (CFE)
  • Litigation Support
  • Recovery

When the numbers stop making sense, we reconstruct the picture. INTEGRIT traces assets, follows financial trails, and documents fraudulent activity so you and your counsel have a defensible basis for recovery, litigation, or a quiet resolution.

Where a matter calls for forensic accounting, INTEGRIT retains Certified Fraud Examiners (CFEs) as part of our 400+ investigator alliance. The CFE conducts the forensic accounting examination — reconstructing books, tracing funds, and quantifying loss — while INTEGRIT's investigators run the field, records, and OSINT work, so you get one coordinated engagement rather than two vendors.

What's involved

  • Asset identification and tracing — real property, business interests, and accounts of record
  • Financial-history reconstruction from lawful public and proprietary sources
  • Fraud-scheme analysis and documentation
  • Forensic accounting by retained CFEs — books-and-records reconstruction, funds tracing, and loss quantification
  • Judgment-recovery and pre-litigation asset checks
  • Litigation and expert-witness support coordination

Typical engagements

  • Suspected embezzlement or internal fraud
  • Collections and judgment enforcement
  • Divorce and hidden-asset matters

Regulatory Brief // FINRA Rule 3290

The new outside-activities rule narrows what your reps must disclose — and widens what your firm must detect.

On September 15, 2026, the SEC approved FINRA Rule 3290 (Outside Activities Requirements), consolidating and replacing Rule 3270 (Outside Business Activities) and Rule 3280 (Private Securities Transactions). The new rule focuses supervision on investment-related activity and removes FINRA's baseline notice and assessment requirements for non-investment outside activities. FINRA has not yet set an effective date — Rules 3270 and 3280 remain in force until it does — but the shift is coming, and the time to prepare for it is before it lands.

In a hybrid RIA/OSJ model, the broker-dealer has long caught undisclosed outside business and "selling-away" risk through the broad disclosure net of Rules 3270 and 3280. As that net narrows to investment-related activity, the obligation moves from the representative's duty to disclose to the firm's duty to detect. Supervisory systems and written supervisory procedures under FINRA Rule 3110 now carry the weight of identifying what registered persons are no longer required to volunteer. Adopt the lighter baseline without independent diligence, and the blind spot opens exactly where undisclosed advisory activity and outside dealing live.

Outside-activity disclosures that reps once filed as a matter of course may simply stop appearing.
The firm — not the representative — now owns the risk of missing an undisclosed outside engagement.
Rule 3110 procedures and the OSJ supervisory calendar need independent verification built in.
Dually-registered advisers in the RIA/OSJ overlap are where the exposure concentrates.

INTEGRIT's investigative diligence closes that gap: independent verification of outside activities, undisclosed affiliations, and selling-away risk, built to sit alongside your Rule 3110 supervisory system rather than replace it.
